Student Vaccination Opportunities

Dear Hirsch Families:

As you may have heard in the news, the Pfizer vaccine for COVID-19 has been approved for use in children ages 12 and older. For those who are interested, we are available to assist students in this age group, and their family members, register for a vaccine clinic. This is available for both in-person and virtual students. Parents may accompany 12-17-year old to the clinic or fill out a parent consent form.

If you are interested in registering your child for upcoming vaccination clinics, please respond to the link below and you will be provided opportunities to select the hour that is preferable for you.  If bus transportation is needed, then those appointments will have to be scheduled for 4 p.m.  If you would like to participate in the clinic as well, or if you have family members who would like to participate, you may also register using the link provided. These are available on a first-come, first-serve basis.

Registration link and consent forms (you will need Student ID number):

Note: While proof of insurance is requested, it is not required for students.  Furthermore, family members who do not have insurance are still allowed to participate.

Thank you.
